Dental has partnered with Kelly & Associates to bring you the dental care portion of the Evergreen Limited Benefit Health Plan. The Evergreen Benefit Plans all include the Value Dental program, a
denex value product. In addition, there are two fully insured buy-up options for dental coverage that are also offered by
Denex Dental.
The Quality and Comprehensive Plans are the dental insurance buy-ups offered in the Evergreen Benefit Plan suite of products. These plans include fixed reimbursements for specified dental procedures. Members of these plans have the opportunity to combine their Quality or Comprehensive reimbursements with Value discounts to maximize savings and reduce out of pocket expense.

Value dental is not insurance. It is a discount program. You are responsible for the full cost of any health care services purchased. You will receive discounts for medical services at certain health care providers who have contracted with the plan. denex value does not make payments directly to health care providers.
